I hadn’t thought to check for a duplicate initially because it wasn’t showing as a Probable Duplicate Submission. It wasn’t until the designer uploaded another design that I knew right away was a duplicate that I decided to check the first design they uploaded. And sure enough, it was a duplicate as well.

I didn’t realize this person created a second account (or so it seems) and uploaded the design to both accounts. I had checked the deleted tab and saw that it had been deleted once already, so thinking that it was the first one I approved that had been deleted, I approved the second one. Sorry!
